The 5-Year Limited warranty is null & void in the event of misuse or abuse of the Polycarbonate Dinnerware.
This form must be filled out in its entirety and submitted to Carlisle FoodService Products within fourteen (14) days after date of purchase for the limited warranty to be honored.
